We are happy to announce that Dr. Nirupama Gupta has received designation as a Civil Surgeon and is authorized by the US Citizenship and Immigration Services to perform Immigration Physicals for the Department of Homeland Security.
Certified and Sealed I-693 form along with a copy for your records.
Please note that any additional required laboratory, radiology, and vaccination would be charged extra.
Please bring the following to your appointment:
Valid government-issued picture ID, even for children (i.e. passport, driver’s license, employment authorization, learner’s permit, state ID); please note that school IDs do not qualify
Immunization records, if available (i.e. children’s vaccination records); please note: we do not call for records. During flu season (October-March), bring in a flu record.
Medical records, if relevant (i.e. prior treatment for tuberculosis)
Laboratory studies, if available: Quantiferon Gold or T-Spot (TB blood test); RPR and Gonorrhea (both not older than 1 year) for ages 15-and-older; measles/mumps/rubella and varicella titers.
The following are required to complete the I-693 form:
Physicalexamination (includes review of health history and brief exam); please bring your eyeglasses!
Tdap (tetanus vaccine), not older than 10 years (please note that Td is not acceptable). Please note: if you are a woman who has given birth in the past ten years, your OB/GYN office has a Tdap record on file (Tdap injection is given in the third trimester).
Quantiferon Goldor T-Spot (Tuberculosis) blood testing for all age groups including children
Chest x-ray (necessary only when TB test is positive; the x-ray must not be older than 1 year)
Syphilisblood testing (RPR) is required for ages 15-and-older (should not be older than 1 year)
Gonorrheaurine test is required for ages 15-and-older (should not be older than 1 year)
MMR titers if born in 1957 or later (not needed if immunization record of two MMR vaccines)
MMR vaccine if titer is not immune.
Varicellatiter (not needed if history of chicken pox disease or immunization record of two varicella vaccines)
Varicella vaccine if titer is not immune.
Fluvaccine (during flu season [October to March].
For ages 65-and-older, Pneumococcalvaccine [Pneumovax23]
For ages 17-20, Meningococcal vaccine.
For ages 18-and-under, childhood vaccination records.
Immigration exams and/or vaccinations will not be covered by insurance companies.
If you are pregnant please let the physician know so that we can administer the appropriate vaccinations.
If additional information or papers are needed, it will take longer then the regular processing time.
All paper work will be ready within 7-10 days for you to pick up.
