Immigration Physicals

Dr. Nirupama Gupta is designated as a Civil Surgeon and is authorized by the US Citizenship and Immigration Services to perform Immigration Physicals for the Department of Homeland Security for ADULTS AND CHILDREN.

Report of Medical Examination and Vaccination RecordIf you are applying for adjustment status to become a lawful permanent resident, use this form to establish that you are not inadmissible to the United States on public health grounds.

The Cost of the Immigration Physical includes:

Physical Examination & Labs (gonorrhea, syphilis, tuberculosis, MMR and VZV titers)
Review of health records and vaccination history
Certified and Sealed I-693 form along with a copy for your records.
Please note that any additional required laboratory, radiology, and vaccination would be charged extra.

Please bring the following to your appointment:

Valid government-issued picture ID, even for children (i.e. passport, driver’s license, employment authorization, learner’s permit, state ID); please note that school IDs do not qualify
Immunization records, if available (i.e. children’s vaccination records)

*During flu season (October-March), bring in a flu record.

* Starting October 1, 2021: Must have COVID vaccination proof, if age eligible.

* Starting May 2022: Must have one dose of Hepatitis B vaccine.

Medical records, if relevant (i.e. prior treatment for tuberculosis)
Laboratory studies, if available: Quantiferon Gold or T-Spot (TB blood test); RPR and Gonorrhea (both not older than 1 year) for ages 15-and-older; measles/mumps/rubella and varicella titers.

Please Note:

Immigration exams and/or vaccinations will not be covered by insurance companies.
If you are pregnant please let the physician know so that we can administer the appropriate vaccinations.
All paper work will be ready within 7 days for you to pick up.
